Camper Scholarship and Registration Forms

Camp Au Sable's Matthew Jacobs memorial worthy camper fund was set up for people to be able to donate to help Michigan kids who may need financial support in order to be able to participate in the Camp Au Sable experience. Below you will find more about the paperwork required to apply.

Campership
Request Form

Campership, also known as the Matthew Jacobson Fund, assists campers from Michigan Seventh-day Adventist churches whose families can pay part of the registration fee but need help with the remaining cost.

Campership Recommendation Form

The Campership Recommendation Form allows a church member to provide a recommendation for a camper. This information helps camp staff prayerfully and thoughtfully determine how Campership (Matthew Jacobson Fund) scholarship funds are awarded.

Friendship Camper Program

The Friendship Camper Scholarship is a church-sponsored friendship ministry for children who are not members of the church. Through this program, churches build relationships with families by sponsoring a child’s camp experience. Contact your local church to see if they are participating this summer.

Friendship Campers

Additional Forms

Youth Camp forms are completed as part of online registration and are required for all campers. These include the Consent & Assumption of Risk, Camper Medical Form, Photo Release, Secondary Contact and Authorized Pick-Up form, and a copy of the camper’s health insurance card (front and back).
