Parents/legal guardians:

Register your camper online and fill out all the forms during the online registration process.

Grandparents, relatives, friends – During the online registration process, print the forms and have a parent/legal guardian fill them out. Then choose from these three options:

• Bring them with the child on registration day
• Scan and email the forms 4 days before registration to jramos@misda.org
• Fax the forms Thursday evening by 5pm before registration to 989-348-5194.

If you do not meet this deadline, please bring the documents with you.

Youth Camp Forms:

1. Consent & Assumption of Risk (pdf link)
2. Camper Medical Form (pdf link)
3. Photo Release (pdf link)
4. Secondary Contact and Authorized Pick up (pdf link)
5. Copy of Health Insurance Card (FRONT AND BACK)

[Note: If the camper does not have medical insurance, a written and signed document stating that fact is sufficient.]

Family Camp Forms:

Since there are many members in the family who vary in age, you will need to bring a printed hard copy of the following paperwork. If a minor is attending Family Camp with another family (but without their parents), they must also bring all forms under "Youth Camps" above: Consent & Assumption of Risk form signed by a parent/legal guardian, Camper Medical Form, and copy of insurance card (front and back).
