Camp Forms

To speed up the registration process, print the following forms, fill them out completely, and bring them with you to registration.

Youth Camps:

1. Consent & Assumption of Risk

2. Camper Medical Form

3. Photo Release

4. Copy of Camper Immunization Record

5. Copy of Health Insurance Card (FRONT AND BACK) [Note: If the camper does not have medical insurance, a written and signed document stating that fact is sufficient.]

Family Camps:

1. Family Camp Photo Release

2. Family Camp Consent & Assumption of Risk

*If a minor is attending Family Camp with another family (but without their parents), they must also bring all forms under "Youth Camps" above: Consent & Assumption of Risk form signed by a parent/legal guardian, Camper Medical Form, copy of immunization record, and copy of insurance card (front and back).
